However, there are other potential treatment options such as open excision with cricopharyngeal myotomy that although can have greater potential risk, may confer better long term outcomes. Other options include laser division of cricopharyngeal bar.ObjectiveRetrospective analysis of pharyngeal pouch management at Bradford Royal Infimary. To investigate the outcomes of patients with pharyngeal pouches managed surgically.Results36 cases included. Out of 25 endoscopic stapling of pharyngeal pouch, 15(60%) successful; 4(16%) had partial; 3(12%) temporary success; 1 unknown result; 2(8%) failed. Of 10 open excision, 10(100%) success; 7 discharged after 1st follow up; 1 after second follow up; 1 ongoing follow up due to persistent globus. 1 treated with laser of bar had temporary success.ConclusionHigh success rate of open surgery with better long-term outcomes. Complication rates comparable to other studies.

Abstract : The Tasmanian tiger or thylacine ( Thylacinus cynocephalus ) was an iconic Australian marsupial predator that was hunted to extinction in the early 1900s. Despite sharing striking similarities with canids, they failed to evolve many of the specialized anatomical features that characterize carnivorous placental mammals. These evolutionary limitations are thought to arise from functional constraints associated with the marsupial mode of reproduction, in which otherwise highly altricial young use their well-developed forelimbs to climb to the pouch and mouth to suckle. Here we present the first three-dimensional digital developmental series of the thylacine throughout its pouch life using X-ray computed tomography on all known ethanol-preserved specimens. Based on detailed skeletal measurements, we refine the species growth curve to improve age estimates for the individuals. Comparison of allometric growth trends in the appendicular skeleton (fore- and hindlimbs) with that of other placental and marsupial mammals revealed that despite their unique adult morphologies, thylacines retained a generalized early marsupial ontogeny. Our approach also revealed mislabelled specimens that possessed large epipubic bones (vestigial in thylacine) and differing vertebral numbers. All of our generated CT models are publicly available, preserving their developmental morphology and providing a novel digital resource for future studies of this unique marsupial.

Background: Pharyngeal pouches are an uncommon cause of dysphagia, managed via open or endoscopic techniques. Objectives: This retrospective audit identifies long-term outcomes of endoscopic surgery, evaluating the virtues of a subspecialist service. Methods: Study approval was obtained; patients were identified within the NHS and private sector within Worcestershire. Consent was obtained for telephone interviews/postal surveys and case notes reviewed. Results: Forty-seven patients (fifty episodes), after exclusions, underwent endoscopic stapling procedures between September 1997 to April 2014. Outcomes included: patient satisfaction ratings, symptom changes, open surgery conversion and complications. Study follow-up varied, extending up to 17-years post-operatively. Conversion to open surgery was 5.4%; an improvement from previously published audits. Conclusion: Our study suggests endoscopic management of pharyngeal pouches is safe and effective and that patient selection has improved associated with an improvement in the surgical learning curve aided by service sub-specialisation, currently largely delivered by the senior author within the region.
