"Approximately 80% of subfertile couples still have a chance to conceive naturally and the most important thing is to assess whether any form of medical assistance will increase their chance of pregnancy over continuing natural conception. In couples with male subfertility, IUI, IVF and ICSI are frequently used fertility treatments. Evidence-based guidelines on male subfertility are limited and the use of medically assisted reproduction is generally experience-based. In roughly 1% of subfertile couples the male partner is diagnosed with non-obstructive azoospermia. Men with NOA are able to father their own genetic child by using testicular sperm extraction (TESE) in combination with ICSI. Since both TESE and TESE-ICSI are invasive and costly procedures, patients should be well informed about their chances of successful TESE and their likelihood to conceive after TESE-ICSI before undergoing treatment. In this thesis, we performed several studies on male subfertility and MAR. We concluded that categorizing couples with male subfertility based on the TMSC allows for better classification in terms of chance of spontaneous ongoing pregnancy rate than categorization using the WHO classification system. We also concluded that there is insufficient evidence to determine whether there is any difference in safety and effectiveness between different treatment strategies for male subfertility. Furthermore, we developed a prediction model for obtaining sperm after testicular sperm extraction (TESE) in men suffering from NOA, as well as a prediction model for live birth in TESE-ICSI. Finally, we concluded that the current sperm DNA fragmentation tests have limited capacity to predict the chance of pregnancy in context of MAR."--Samenvatting auteur.

The management of infertility using acupuncture is an expanding area of practice and one which is frequently rewarding for TCM acupuncture practitioners. Acupuncture for IVF and Assisted Reproduction has been specially prepared to meet the growing demand for information in this area and draws upon 20 years combined experience of the authors together with the latest evidence from both orthodox medicine and TCM. Richly illustrated and clearly written throughout, the book takes the reader through the anatomy and physiology of reproductive medicine (from both an orthodox and TCM perspective) and explains the underlying basis of orthodox medical fertility tests and investigations. The volume then explores the pathology and aetiology of TCM syndromes and shows how common fertility-related conditions, such as endometriosis and male factor infertility, affect Assisted Reproductive Technology (ART) success rates. It explains in great detail how to take a reproductive medical history and successfully diagnose TCM syndromes. Acupuncture for IVF and Assisted Reproduction also provides guidelines on how to regulate the menstrual cycle in preparation for IVF treatment and shows how lifestyle can affect fertility and ART success rates. Placing a strong emphasis on the practical aspects of patient care, Acupuncture for IVF and Assisted Reproduction contains an abundance of case history templates, algorithmic acupuncture treatment pathways and patient fact sheets and will be ideal for all acupuncture practitioners working in this field. "A must have for the bookshelf of any acupuncturist who is ever called upon to treat fertility issues - if you have room for one book this surely must be it." Reviewed by The Acupuncture Fertility Centre March 2015 "Practitioners of all levels of experience and TCM students should find it compelling reading and an invaluable companion to their learning." Reviewed by Stephen Clarke, Journal of the Australian Traditional Medicine Society May 2015 "This book is extremely well re-searched and referenced." This practical, extensively illustrated handbook covers the procedures that are undertaken in andrology and ART laboratories to analyse and assess male-factor infertility, and to prepare spermatozoa for use in assisted conception therapy. The content is presented as brief, authoritative overviews of the relevant biological background for each area, plus detailed, step-by-step descriptions of the relevant analytical procedures. Each technical section includes pertinent quality control considerations, as well as the optimum presentation of results. In addition to the comprehensive 'basic' semen analysis, incorporating careful analysis of sperm morphology, the handbook provides established techniques for the use of computer-aided sperm analysis and sperm functional assessment. Throughout the handbook the interpretation of laboratory results in the clinical context is highlighted, and safe laboratory practice is emphasized. It is an invaluable resource to all scientists and technicians who perform diagnostic testing for male-factor infertility.
