The treatment of many rheumatic diseases has changed dramatically in the last 10 years with advances in our understanding of the pathophysiology and the availability of a whole range of new drug therapies. During that time, approaches to the optimum management of an individual patient have also been facilitated by an increasing awareness of the importance of evidence based medicine with the requirement for a weighing of the evidence for individual treatments to ensure the best possible decision for our patients. Although many rheumatologists will be familiar with many of these advances in drug therapy, knowledge of when to integrate such therapies with adjunctive and ancillary therapies, especially from an evidence based medicine point of view, is not always so easy to obtain. Moreover, at the same time as there have been advances in drug therapy, there have also been advances in our understanding of the role of such ancillary measures such as patient education, physiotherapy, occupational therapy and podiatry as well as the place of surgery in chronic rheumatic diseases.
