Autopsy and structural MRI studies have demonstrated that the most consistent site of neuroanatomic abnormality in autism is the cerebellum. An important question is how such pathology impacts cerebellar function. Traditional views hold that the cerebellum is dedicated to motor coordination, but recent functional MRI studies have challenged this view. One such study (Allen, G., Buxton, R.B., Wong, E., & Courchesne, E., 1997) demonstrated a dissociation between regions involved in attention and those involved in a motor task. Motor activation was localized to the anterior cerebellar hemisphere, while attention activation was localized to the superior posterior hemispheres. The present investigation examined activation in the cerebella of autistic patients and normal controls performing these same tasks. The prediction was that reductions in viable cerebellar tissue due to an early reduction of Purkinje cells would result in abnormally reduced activation. An anatomical region-of-interest (ROI) approach was employed. Eight patients with autism and eight normal controls matched for age, sex, handedness, and scores on the Wechsler Block Design and Object Assembly subtests participated. For the motor task, subjects pressed a button with the dominant thumb repeatedly at a comfortable pace, and activation was compared to rest in four ROIs: anterior vermis (AVe), anterior/posterior verinis (A/PVe), ipsilateral anterior hemisphere (iAH), and ipsilateral hemisphere lobule VI (iVI). Autistic individuals had significantly increased activation in A/PVe, iAH, and iVI. Exploratory analyses also showed increases in contralateral and posterior cerebellar regions. During the attention task, circles, squares, or triangles in red, green, or blue were presented one at a time at fixation. Subjects pressed a button to every target (e.g., any red shape). Activation during this task was compared to activation during passive visual stimulation in four ROIs: iVI and contralateral VI, and right and left hemisphere lobules VIIA. As predicted, autistic subjects had reduced activation relative to normal in all regions of the cerebellum. This group effect was particularly prominent after controlling for motor activation. These findings strongly suggest that the cerebellum is functioning abnormally in autism, lending further support to the role of cerebellar abnormality in this common and devastating neurodevelopmental condition.
